Your First Visit at the Dentist

Your First Visit Will Be An Initial Consultation and Oral Examination

If you're scheduling your first visit, we ask that you relax. Your first visit will be an initial consultation and oral examnination.We have everything you need right here to make your experience worthy of a smile.

Prior to your initial consult, we will need the following:

Referral slip from your general dentist
Any x-rays that have been taken in the last year
Your dental and medical insurance card
A brief health history (this will be taken over the phone at the time you make your appointment & an in-depth history will be taken a the time of your consult.)

We must have a current panoramic x-ray (one that has been taken over the past year) in order to accurately assess your condition and develop an appropriate treatment plan. If you do not have any x-rays, our facility is equipped to take them during your consultation.

Please note: All patients under the age of 18 must be accompanied by a parent/guardian for each visit.

Fill Out Your Forms and Send Them Prior to Your Visit

We here at Cusumano Oral Surgery and Implant Center are going green by reducing our use of paper. Please download the forms to digitally sign them, then simply upload the signed agreements with purple file button that says "Send us a File" on the left hand side of your screen. Below are instructions as to how to digitally sign your patient forms.

2. Open for in Adobe Reader or Acrobat. (Reader is free)

3. Click the Tools button on the top left and select Sign & Certify

4. Once you have signed the document. Please Save and Upload using the Send Us a File button.
